You can go click on GET CERTIFICATES and complete the certificate yourself. This can be accessed 24 hours a day 7 days a week. If the venue needs special wording, you can request the certificate in writing by submitting the certificate request form found under FORMS on our welcome page. Our fax number and email address are listed on the form. These will be completed as soon as possible during normal business hours.
