Why are my dropdown menus reverting back after making a selection when previewing?

Setup > Event Planning > Worksheets - Select a worksheet.  Locate "Actions" section.  Select "Preview Form" from the dropdown menu.




In the popup window, you can select choices in dropdown menus but they will revert back to the top default choice.  This only happens when previewing the form from within GigBuilder.  This does not occur when viewing the worksheet as a client.
